A New Chapter in Care: Continuous Glucose Monitoring (CGM) Program

Over time, one need has become increasingly clear.

We are serving more and more patients living with chronic conditions—especially diabetes. Many are working hard, doing everything they can to manage their health, but without insurance, they face real barriers to consistent care, education, and the tools needed to stay healthy.

That’s why we launched our Continuous Glucose Monitoring (CGM) Program.

Why a CGM Program?

For many patients, managing diabetes has traditionally meant finger sticks, guesswork, and limited insight into how their body responds throughout the day.

CGMs change that.

These devices provide real-time glucose data—giving patients and providers a continuous picture of what’s happening in the body. Instead of reacting to problems, patients can begin to understand patterns, make adjustments, and take proactive steps toward better health.

For our patients, this is more than technology.
It’s clarity.
It’s confidence.
It’s the ability to take ownership of their health in a way that simply wasn’t possible before.

The patients we serve are already navigating life without insurance—and too often, that means delaying care until something becomes urgent. For someone living with unmanaged diabetes, that can be incredibly dangerous. One complication—or even a single emergency room visit—can quickly become not just a medical crisis, but a financial one as well.

That is one of the reasons this program matters so deeply.

By equipping patients with real-time data, education, and support, we can help them manage their condition earlier, avoid preventable complications, and reduce the risk of a crisis that impacts both their health and their financial stability.

What Makes Our Program Different

This isn’t just about handing someone a device.

We have built a comprehensive, patient-centered program designed to surround each individual with the support they need to succeed.

Our CGM Program includes:

  • Real-time glucose monitoring using advanced CGM devices

  • Education and training to help patients understand and use their data

  • Volunteer-led support, including individuals trained to walk alongside patients

  • Nutrition guidance, working with partners and nutritionists to connect food choices with real outcomes

  • Ongoing care coordination with our medical team

This is about connecting the dots—helping patients not just see their numbers, but understand them, respond to them, and improve their overall health.

How We Made It Happen

Like everything at Hope, this program is built on community.

It exists because of:

  • Generous grant funding

  • Support from local businesses and organizations

  • Dedicated volunteers willing to step in and serve

  • Strong community partnerships

We are especially excited about the role volunteers will play—helping provide CGM education, encouraging patients, and supporting lifestyle changes that lead to lasting results.

What We’re Seeing Already

We are still early in this journey—but the impact is already clear.

We are seeing patients:

  • Gain a better understanding of their health

  • Make meaningful changes in their daily habits

  • Experience improved glucose control

  • Feel encouraged, supported, and hopeful

One patient shared, “It is so nice to have real-time data and not have to worry about finding transportation to come in for repeated finger sticks.”

That matters.

For many of our patients, this is the first time they have had access to this level of care—and it is already making a meaningful difference.

Looking Ahead

We are very, very excited about what this program will become.

This is just the beginning.

As we continue to grow, we are actively seeking:

  • Additional funding through grants and community partners

  • Business support to expand access

  • Volunteers who are passionate about education, health, and serving others

Because the need is real—and the opportunity to make a difference is even greater.
